INSURANCE BENEFITS VERIFICATION

1. Be ready with your insurance card and a way to record the information you will gather. 

 

 

2. Call the member phone number on the back of your insurance card.

3. Speak with a representative to answer the following questions:

• What is the effective date of my insurance policy?  When does my plan renew?

​

• Does my plan cover acupuncture provided by a licensed acupuncturist?

              You may need to provide the following CPT codes: 99204; 99212; 97810; 97811

​

• Is Krista Obenlock, LAc, NPI#: 1669073490 in network with my plan?

               If the answer is no, ask whether your plan has coverage for acupuncture provided by out- 

               of-network acupuncturists.

• Is there a visit limit for acupuncture?  How many visits do I have remaining this year?

​

• Are acupuncture benefits split between another service (for example, chiropractic)? ​

​

• What is my copay or coinsurance for acupuncture office visits? 

​​

• What is my deductible?  Does it need to be met in order for insurance to pay for acupuncture? 

​​

• Is a referral or pre-authorization needed in order to receive acupuncture?  If so, how do I obtain this?​​​​​​​

​

4. Ask for a call reference number and write it down.
